Botox treatment for chronic migraine questionnaire

Migraine Action in conjunction with Dr Dowson, Chairman of their Medical Advisory Board have asked me to pass this questionnaire on to all those who have tried Botox.

It is being collected to help Migraine Action prepare its feedback with regard to the forthcoming NICE review of Botox as a treatment for chronic migraine, they are looking to get information from those who have tried the treatment for their chronic migraine / headache (which followed the PREEMPT study protocol by the manufacturers) and was either funded privately or on the NHS.

Please help by taking a few moments to complete this survey, the results of which will provide valuable supporting information.

Please think of a typical 4 week period when answering the questions.
